RISK OR BENEFiT DURING PSORIASIS TREATMENT: SUN OR PUVATHERAPY ?

The Psoriasis is a dermatologic disease with a chronic evolution, on whose origin several factors, environmental, dietetic, standard of living, traumatic, pharmacological, genetic, etc., seem to intervenue. More recent researches have identified on six chromosome, the site of Major Histocompatibility Complex (MCH) in which the TNF, 'the BAT, the segment of Heath Shock Proteins (HSP) and, above aIl, the antigenics classes of the HLA complex of class I (A,B,C),that regulate the transplant antigenics, are incluted. In these years the researches that associate some formes of psoriasis to very precise antigens classes of the HLA complex such as HLA-CW6, HLA-DR7, HLA-BI3, etc. are numerous. The identification of the high genetic component, at least for the psoriasis type I,justifies t1le use ofdrugs that interfere patient's immunity response (Cyclosporin, Met1lotrexate) or that interfere t1lfOUgll various mechanisms (retinoids, psoralens, etc. ). Some of these therapies have demonstrated capacities to cause cancer both if they are used alone (Methotexate) and, above aIl, if they are associated to treatment with ultraviolet radiation produced by special lamps (Methotrexate and PUVA, Cyclosporin and PUVA).

The authors wonder if a potential mortal therapy to treat_a-non-risky for life disease is deontological. They suggest a major use of the solar therapy and of particular peloids (slimes and peats) applied with the "shirt" method. With these practices it would be possible not only to extend the treatment season of the thalassotherapics centers in the seasons and in the days with non-sun, but, above aIl, to avoid the appearance of the cutis cancer described during the PUV A therapy.
